  • It is a secure online website that will give you convenient access to your personal health information, medical records, and forms. You can use the Patient Portal on any device with an Internet connection.

  • If you have not used the Patient Portal before, please call 503-238-5203 and we would be happy to set you up with the information needed to get into your Patient Portal.

  • If your Portal has been enabled, you will receive an email with the steps on how to set up the Portal. The information sent is only valid for 72 hours (about 3 days) from the time it was sent. Please call 503-238-5203 if you need this resent to you.

  • Please check your inbox and your spam, if you still don’t see it there, please contact our offices at 503-238-5203.

  • Below the log in information, click “forgot password.”

    Username will be your email address

    Once all fields are filled out, an email will be sent to you with your password reset

    here you will find all the documents that will need to be completed.
