    • The Service Interest form can be filled out by any of the following:

    • Self-referrals (I myself am interested in accessing services at Quest Center.

    • Outside Agency Referral (I am interested in connecting a client that I work with to services at Quest Center.)

    • Quest Center Staff Referral (I work at Quest Center and am filling this out for a current, past, or prospective Quest client.)

  • No. While Quest Center is committed to servicing the LGBTQIA2S+ community, we are not exclusive to the LGBTQIA2S+ community. Quest Center welcomes all identities and sexualities. We lean into the intersections of the community that we serve.

  • We will follow up within 3-5 business days with more information and next steps.

  • More than 70% of our clients are medically un-insured or under-insured. Most Oregon Health Plan (OHP) and many other commercial insurance plans are welcomed at Quest Center. 

    Different insurance plans offer varying levels of coverage for treatment. It's important to check with your insurance provider to understand what is covered under your plan. Understanding your benefits can help minimize out-of-pocket expenses.

  • If you don’t have insurance, don’t let that stop you from reaching out! Quest Center has staff trained to assist with enrollment in the Oregon Health Plan. We also have a sliding fee schedule for anyone who may need to pay out of pocket.
